(SBSM)
Prevent. Promote. Protect.
SBSM is a government program providing services to pregnant or parenting women.
SBSM offers:
- Intensive case management lasting until the women's youngest child reaches age 3.
- Services to women in residential treatment facilities, transitional housing,
and independent home environments.
- Outreach to women who are contemplating treatment but not yet ready to engage in services.
- Experienced staff, including a public health nurse and two behavioral health specialists.
- Government funding to assist with obtaining services.
Services
SBSM facilitates family-oriented early intervention services in order to help the family achieve an
independent, healthy, drug-free lifestyle. Based on individual needs,
a family may be connected with the following services:
- Parent education, using the 'Make Parenting a Pleasure' Curriculum
offered through Benton Franklin Health District.
- Child development assessments using Denver and NCAST screening tools.
- Theraputic daycare.
- Chemical dependency treatment, either inpatient or outpatient,
as recommended by the Substance Abuse Assessment Center.
- Education.
- Job skills, including employment training, resume writing, and interviewing techniques.
- Healthcare.
- Appropriate housing.
Program Eligibility
In order to qualify for the SBSM program, women must meet the following eligibility requirements:
- 18 years of age or older.
- Pregnant and/or parenting a child under 3 years of age.
- residing in Benton-Franklin County.
- At risk for drug and/or alcohol abuse.
- Must be Medicaid eligible.
Women who do not qualify for the SBSM program
or who choose not to participate will be referred to other services.
Referral Process
Eligible women may be referred by a variety of sources, including the following:
- Full name of person being referred
- Date of Birth
- Complete address and phone number
- If being referred from treatment center, must permanently reside in Benton-Franklin counties
- Her or her child(ren) have medical coupons and can provide proof at enrollment
- Estimated due date (if pregnant) or name and date of birth of youngest child
- Name and age of other children
- CPS involved and if so, placement of the children
- Drug of Choice
- Currently in treatment or not
